Description of what S.T.A.R. nurses do:

The primary responsibility of the S.T.A.R. RN is to prepare the patient for discharge. They educate the patient & family about their disease process(es), medications, and home care. They reconcile their medications and set up follow-up appointments. They assure that any home care needs are met thru Social Service. They then discharge the patient.

S.T.A.R. nurses also specialize in patient education for stroke and diabetic patients. When not doing discharges, they visit stroke and TIA patients to begin the education process for each patient and family. They also assess the educational needs and teach diabetic patients during their stay. They speak in the community re: stroke prevention and the need for seeking rapid medical treatment should symptoms occur.

All diagnosis, with a special focus in stroke and diabetes.
